It is the Board’s policy to provide 6th, 7th, and 8th grade students opportunities for a variety of athletic competitions with teams from other schools.
Every pupil wishing to participate in intramural and /or interscholastic athletic activities shall be required to complete a Parent Permission Sports Application Form to request permission to participate in a sport. The parents/guardians of the student shall consent to their child’s sports participation and acknowledge that physical hazards may be encountered.
Medical Examination Requirement
Each candidate for a school athletic team must be examined within 365 days prior to the first practice session. The school shall provide a physical examination for those students who do not have a medical home (private physician). All physicals must be documented on a form furnished by the district. Please be sure that your physician completes all required information on forms. Incomplete forms can not be accepted. Physicals are good for one year from the date the examination.A Sports Physical packet that includes a (1)Parent Permission form, (2)PreParticipation Physical Evaluation form, a (3 required if 90 days have passed since physical was done)Health History Update Questionnaire (completed by the parent/guardian), a signed (4)Concussion Head injury Parent Acknowledgement Form and (5)Sudden Cardiac Death Sign Off Sheet (6) Opiod Fact Sheet sign off sheet must be turned in by the deadline for that season. The deadlines for each sport season are printed on the first page of the Sports Physical Packet. Forms can be found online under "Forms" on this school nurse page, or in the nurses office. Sports Physical forms found on this website are updated annually. Old forms will not be accepted.
